MOHS Surgery is a precise, microscopically controlled technique used to remove common skin cancers, such as basal cell carcinoma and squamous cell carcinoma. It involves removing thin layers of cancerous tissue one at a time, examining each layer under a microscope, and continuing the process until no cancer cells remain.

The MOHS Procedure: A Step-by-Step Approach
The MOHS procedure is characterized by its meticulous, layer-by-layer approach to ensure complete cancer removal while minimizing damage to surrounding healthy skin.
Initial Surgical Removal
The process begins with the surgical removal of the visible tumor, along with a very thin margin of surrounding skin. This initial layer is carefully mapped to track the precise location of the cancerous cells.
Microscopic Examination
Following the initial removal, the excised tissue is immediately sent to an on-site laboratory. Here, each layer is meticulously processed and examined under a microscope by the Mohs surgeon. This critical step allows for the identification of any remaining cancer cells at the edges or within the removed tissue.
Further Tissue Removal (if needed)
If cancer cells are detected during the microscopic examination, another thin layer of tissue is removed, specifically from the area where the cancer was identified. This new layer is then also examined under the microscope.
Continuation Until Clear Margins are Achieved
This cycle of tissue removal and microscopic examination continues until all margins of the excised tissue are microscopically free of cancer cells. This ensures that the entire tumor has been removed.
Same-Day Reconstruction (if needed)
Once clear margins are confirmed, the wound is ready for reconstruction. Depending on the size and location of the removed tissue, Dr. Schlam can perform immediate reconstruction, often on the same day as the surgery. This may involve simple wound closure, skin grafting, or other reconstructive techniques to restore both function and appearance.

Recovery and Aftercare
Following the procedure, most patients experience mild swelling and redness, which subside within a few days. Stitches, if used, are typically removed within a week. Dr. Schlam provides detailed post-operative instructions to ensure optimal healing and recovery.
