Topical minocycline is a prescription medication available in different formulations, such as Amzeeq (foam) and Zilxi (foam), used for the treatment of specific skin conditions. Amzeeq is indicated for the treatment of certain types of acne in adults and children aged 9 years and older. Zilxi is prescribed for the treatment of inflammatory lesions associated with rosacea in adults.
Minocycline belongs to a class of drugs known as tetracycline antibiotics. These antibiotics work by inhibiting the growth of bacteria, which can contribute to acne and other skin infections. While the exact mechanism of how Amzeeq works to treat acne is not fully understood, it is believed to possess anti-inflammatory properties that help reduce redness and swelling associated with inflamed acne.

Understanding Topical Minocycline
Topical minocycline is applied directly to the skin, targeting the affected areas. It is crucial to follow the prescribed dosage and application instructions carefully. Using the medication around the same time each day can help maintain consistent levels of the drug on the skin.
Available Formulations and Dosage Forms
- Amzeeq is available as a 4% topical foam that is applied to the skin.
- Zilxi is also a topical foam formulation used for rosacea.
It is important to consult with your doctor or pharmacist if you have any questions or require clarification on how to use the medication.
How to Use Topical Minocycline
Always use topical minocycline exactly as directed by your healthcare provider. Before applying the medication, ensure the foam can has warmed to room temperature, especially for the first use. Shake the can well before each application.
After applying the minocycline foam to the affected skin, wash your hands thoroughly. Avoid getting the medication into your eyes, mouth, the corners of your nose, or the vaginal area. Do not bathe, shower, or swim for at least one hour after application.
Missed Doses
If you miss a dose, apply it as soon as you remember. However, if it is nearly time for your next scheduled application, skip the missed dose and resume your regular dosing schedule. Do not apply extra foam to compensate for a missed dose.
Precautions and Warnings
Before starting treatment with topical minocycline, it is essential to inform your doctor and pharmacist about your complete medical history and any allergies you may have.
Allergies
Do not use topical minocycline if you are allergic to minocycline, sarecycline, doxycycline, omadacycline, tetracycline, demeclocycline, or any other ingredients in the specific minocycline foam product you are prescribed. Ask your pharmacist for a list of ingredients.
Drug Interactions
Inform your doctor and pharmacist about all the medications you are taking, including prescription and over-the-counter drugs, vitamins, nutritional supplements, and herbal products. This is crucial to avoid potential interactions that could affect the efficacy of minocycline or increase the risk of side effects.
Specific medications that may interact with topical minocycline include:
- Blood thinners
- Penicillin antibiotics (e.g., amoxicillin, penicillin V potassium)
- Isotretinoin (used for severe acne)
- Antacids containing aluminum, calcium, magnesium, or iron
- Retinoids
- Ergot medications for migraines
Pregnancy and Breastfeeding
It is important to discuss with your doctor if you are pregnant, plan to become pregnant, or are breastfeeding. While it is not fully known if topical minocycline can harm an unborn baby, oral minocycline has been associated with serious side effects on bone growth and teeth in fetuses. If you become pregnant while using topical minocycline, contact your doctor immediately.
It is also unknown if topical minocycline passes into breast milk. Consult your healthcare provider if you are breastfeeding or plan to do so.
Other Health Conditions
Inform your doctor about any pre-existing health conditions, particularly:
- Kidney problems
- Liver problems
- Diarrhea
- History of increased pressure around the brain (intracranial hypertension)
- Asthma
- Lupus
- Being overweight
Photosensitivity
Topical minocycline can make your skin more sensitive to sunlight, sunlamps, and tanning beds. This condition is known as photosensitivity. Exposure to UV light, even for short periods, can cause severe sunburn, blistering, or swelling. If you need to be in the sun, wear sunscreen with an SPF of at least 15 (or higher if you have fair skin), wear a hat, and wear loose-fitting clothing that covers your skin. Avoid sunlamps and tanning beds.

Intracranial Hypertension
Topical minocycline may increase the pressure around the brain, a condition known as intracranial hypertension (or increased intracranial pressure). This is more likely to occur in women of childbearing age who are overweight. Symptoms may include severe headache, blurred vision, double vision, vision loss, nausea, and vomiting. Contact your doctor immediately if you experience these symptoms.
Autoimmune Syndromes
In rare cases, minocycline can trigger autoimmune syndromes, such as drug-induced lupus-like syndrome, autoimmune hepatitis, vasculitis, and serum sickness. Seek immediate medical attention if you develop a fever, rash, joint pain, or unusual body weakness.
Hyperpigmentation
Minocycline, especially when taken orally, can cause darkening of the skin, nails, bones, eyes, and the inside of the mouth (including teeth). While topical application may have a lesser effect, report any noticeable darkening of your skin or other body parts to your doctor.
Infection Risk
The use of topical minocycline may lead to an overgrowth of certain germs, such as fungi. If your acne does not improve or worsens, or if you develop symptoms like fever or skin inflammation, consult your doctor.
Potential Side Effects
Like all medications, topical minocycline can cause side effects, although not everyone experiences them. Some side effects are mild, while others can be serious.
Common Side Effects
The most common side effect associated with Amzeeq is headache. Other potential mild side effects may include:
- Redness
- Dryness, itching, or skin peeling at the application site
- Flushing of the face or blushing appearance
If any side effects are bothersome or persist, consult your healthcare provider.
Serious Side Effects
Seek immediate medical attention if you experience any of the following serious side effects:
- Severe Allergic Reactions (including DRESS): Symptoms may include breathing problems, wheezing, racing heart, hives, fever, swollen lymph nodes, swelling of the face, lips, mouth, tongue, or throat, shortness of breath, trouble swallowing, tightness in the throat, swelling in the feet, ankles, or legs, nausea, vomiting, dizziness, lightheadedness, fainting, stomach cramps, joint pain, dark-colored urine, or yellowing of the skin or whites of the eyes (jaundice).
- Antibiotic-Associated Diarrhea (C. diff-associated diarrhea): Symptoms can include watery diarrhea, diarrhea that does not go away, blood in the stool, severe stomach cramps, or fever. This can occur during treatment or up to two months or more after stopping the medication. Do not take anti-diarrheal medication without consulting your doctor.
- Liver Damage (Hepatotoxicity): Symptoms may include nausea, vomiting, stomach or belly pain, fever, weakness or unusual tiredness, itching, loss of appetite, light-colored stool, dark-colored urine, or yellowing of the skin or whites of the eyes.
- Dizziness or Lightheadedness: Do not drive or operate machinery until you know how the medication affects you.
- Increased Pressure Around the Brain (Intracranial Hypertension): Symptoms include dizziness, nausea, vomiting, severe headache, or vision changes.
- Autoimmune Syndromes: Symptoms may include fever, rash, joint pain, or unusual tiredness.
- Severe Skin Reactions: These can be life-threatening and include blistering or peeling of the skin, chest pain, dark urine, fever, hives, irregular heartbeat, skin rash, sores in the mouth, stomach pain, swelling in the legs, ankles, or feet, swelling of the face, eyes, lips, tongue, or throat, trouble breathing or swallowing, or yellow eyes or skin.

Storage and Disposal
Storage
Store topical minocycline foam at room temperature, below 77°F (25°C). Do not refrigerate. Keep the container away from fire, flames, and extreme heat. Do not puncture or incinerate the container, as it is flammable.
Keep all medications out of sight and reach of children. Many containers are not child-resistant, so always lock safety caps and store medications in a safe location, high up and out of their sight and reach.
Discard any unused product 90 days after receiving it from the pharmacy.
Disposal
Do not flush topical minocycline down the toilet or pour it down a drain. Instead, use a medicine take-back program. Talk to your pharmacist about available take-back programs in your community. If a take-back program is not available, follow proper disposal guidelines to ensure pets, children, and others cannot access the medication.
Monitoring Treatment
It is essential for your doctor to check your or your child's progress at regular visits to ensure the medication is working effectively. Blood tests may be required to monitor for any unwanted effects. If your or your child's symptoms do not improve within a few days, or if they worsen, consult your doctor.
Before undergoing any laboratory tests, inform your doctor and the laboratory personnel that you are using topical minocycline, as it may affect certain test results.

Important Considerations
- Do not use cosmetics or other skin care products on the treated areas without consulting your doctor.
- Do not take other medications, including prescription or over-the-counter drugs and herbal or vitamin supplements, unless discussed with your doctor.
- Keep a written list of all medications, vitamins, minerals, and dietary supplements you are taking. Bring this list with you to all medical appointments.
- Do not let anyone else use your medication.
